Dear Parents,
Thank you for choosing Catawba Pediatric Associates, P.A. to provide your child’s primary care, and we are looking forward to working with your family. Please take a moment to read this short letter in order to get to know us a little better and to ensure all expectations are met.
Please be sure your insurance information is current at each visit. We will require a copy of your insurance card(s) before services are performed. We will file all insurances in a timely manner. Updated insurance information must be given at the time of service. Failure to do so will obligate you for payment of services rendered. We do not become involved in Third Party liabilities. We do not accept an attorney letter of payment guarantee. If you do not have a copy of your insurance card, you will be responsible for the payment of services at each visit.
If your child is covered under an insurance policy that has a co-payment for services, you are required by your insurance company to pay. Co-pays must be collected at the time services are rendered. Failure to render co-payment at time of appointment will result in non-compliance with your insurance carrier and may be reported to the carrier as designated under our insurance contract. You may also be requested to reschedule your appointment.
If your child is covered under North Carolina Medicaid, you must bring the child’s card to each appointment. If you do not have the card at the time of the appointment, you will be responsible for the charges. If you have private insurance with Medicaid as secondary, we must have a copy of that card as well in order to file your Medicaid. If Catawba Pediatric Associates is not your Carolina Access provider or your Medicaid is inactive, you will be deemed SELF PAY and a minimum payment of $25 will be collected at check-in. Once you advise us and we verify Medicaid has been reactivated, you will receive a refund check.
Good health care for newborns, infants, children, and adolescents begins with the well-child visit (checkup) and other services that help keep children healthy. These are preventive services. Our doctors and staff provide these services based on a plan called Bright Futures. The American Academy of Pediatrics (AAP) made this plan to help doctors and families know what preventive services children should receive from birth to 21 years of age, such as screening tests, and advise about stating healthy and safe. This plan can be altered to suit each child as needed. We also follow the APP vaccine schedule for newborns, infants, children, and adolescents.
There may be times when a child needs a service that is not considered preventive on the same day as a well-child visit. If a child is not well or a problem is found or needs to be addressed during the check-up, the physician may need to provide an additional office visit service to care for the child. This is a different service and is billed to your health plan in addition to the preventive services provided on that same day. If you have a co-payment for office visits or co-insurance/deductible amounts that you must pay before your health plan pays for these services, our office will charge these amounts. We value your time and want to make the most of each appointment for your child.
Negative comments on social media are viewed as a sign that your view and ours no longer align. In this case, we find it in everyone’s best interest to dismiss you from our practice. We will provide you with a records release form and support you in establishing with another pediatrician of your choosing. If you have a negative experience, please contact management immediately.
